You barely see the back of his head so I'm not bothered it wasn't really covered up. I never noticed the arm-thing either, but considering Nicotero took this job simply for the love of it - he even invested a good chunk of his own money into the project, so he wasn't going to have hundreds of staff - considering that I think they did a great job with the effects.
